An exciting opportunity has arisen for an enthusiastic, ambitious and dedicated Senior Physiotherapist to join our newly expanded Acute Community Frailty Integrated Team.

Following a successful pilot of the D2A model during the COVID 19 pandemic, our trust has invested in the expansion of therapy/ nursing services to integrate and enhance acute and community services. D2A model will be focusing upon supporting discharges, avoiding unnecessary hospital admission and providing overarching rehabilitation throughout the pathway.

We are looking for team members to work flexibly across the model and are motivated to drive and develop the pathway.

In return you will receive regular supervision and CPD time, along with being supported by an experienced and dedicated senior management team.
