We have an exciting opportunity for motivated nurses who are enthusiastic about diabetes to join our supportive and dynamic clinical team.

We have an opportunity for;
- Band 6 post diabetes specialist nurse.

What will you do?
The community diabetes multi-disciplinary team consists of specialist nurses, dietitians, GP's and podiatrists who provide care and treatment to patients with Type 2 diabetes. You will be managing your own caseload and liaising closely with primary and secondary care. We need excellent nursing expertise alongside a flair for development within our team. It's our priority to continue delivering the high quality diabetes care for which our service is known. You will have the opportunity to work alongside experienced staff who can help you develop your knowledge, skills and experience.

You must be a registered nurse with a commitment to high quality patient care along with excellent clinical skills and relevant experience. Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
