Following a successful bid for transformation funding, Lincolnshire has been chosen as one of twelve pilots to develop new place based integrated care pathways for individuals with severe and complex mental illness through a neighborhood working approach within four populations; Grantham, Gainsborough, Boston and Lincoln City South.

You will work autonomously in these roles in the context of local and national drivers providing advanced clinical care using high level assessment skills for identification of individual needs which essentially supports diagnosis and treatment options. You will forward or signpost individuals to the most suitable service dependant on your assessment and crucially co-ordinate the patient’s journey across the health, social care system, including voluntary and third sector partners, regardless of care setting.

You will be the designated clinician for named GP practices in a geographical place based area and be responsible for maintaining comprehensive clinical communication and education with general practices and neighbourhood colleagues, providing timely feedback to the service manager.

If you are a registered healthcare professional with evidence of advanced clinical practice to masters level or equivalent and have experience of working autonomously, working in partnership across organisational boundaries and of leading teams and are a values driven practitioner with commitment and desire to make our service the best we can through innovation and collaboration and will have experience of working with adults of working age and/or older adults within mental health needs as we are recruiting for both adult and older adult specialisms.

We offer 24 hour services within our Inpatient, Crisis and SPA teams, if you are successfully appointed to posts in these areas or others that might have specified, you will be required to work unsociable hours and shift patterns.

Preference will be given to NHS colleagues who are currently 'at risk' and meet the minimum criteria of the Job Description & Person Specification.
