Discharge Nurse Job Advert – Ranmore Ward

An exciting opportunity has arisen for a discharge co-ordinator nurse on Ranmore Ward a rehabilitation facility at Dorking Community Hospital.

The successful applicant will be required to facilitate effective, efficient and timely discharge of patients from Ranmore Ward. Particular emphasis is placed upon complex multi-disciplinary assessments including patients with complex health and social care, palliative care or rehabilitation needs. The post holder will be responsible for leading weekly discharge planning meetings and daily white board meetings so as to discuss and plan for successful discharge of each patient, keeping all members of the MDT informed.

The role will involve close working with all members of the multi-disciplinary team (nursing, medical, therapy and social care staff) and patients/relatives/carers, ensuring safe discharge of patients to the most appropriate place. The successful candidate will play a vital role in managing the patient discharge pathway.

We are looking for pro-active, highly motivated individual who will be able to work autonomously and as part of a team. The applicant will be expected to be enthusiastic, organised and have excellent communication and IT skills.

The successful applicant be able to use their own judgement to resolve complex issues. They should be able to work well under pressure and be able to prioritise their own workload under the changing demands of the ward and patients. It is important that any one considering applying for this post will is aware they will be required to keep clinical nursing skills and competancies up to date and at times of pressure on the ward or staff shortages the candidate will be required to work within the staff numbers on the ward clinically.

This role is full time Monday to Friday, applicants who would prefer part time hours will be considered if there are suitable candidates that could successfully job share the role.

Successful applicants, who have not worked for CSH within the last 12 months, will receive an amount of £2000 paid in two instalments: - £1000 upon commencement and a further £1000 following successful completion of probation. These payments are pro-rata for part- time posts.
