Healthcare Assistant - 37.5 hours per week

We have a vacancy for a Band 3 Community Health Care Assistant to join our integrated multidisciplinary team in Responsive Services Central, Mid-Sussex based in Haywards Heath. The service runs between 0800 and 20.00 over 7 days a week. Car driver, essential.

This post would suit an enthusiastic and motivated person who is prepared to meet the changing needs of the population we serve and provide high quality, flexible care where it is needed.

Responsive Services, includes Admission Avoidance, a rapid response service in the community aiming to prevent hospital admissions and keep people in their own home, providing a mixture of emergency support and short term rehab as appropriate. A clinician assesses a patient at home, identifies needs, sets care plans/regular reviews and adjustments, and prepares for discharge planning. In addition, RS provides a Home First service working alongside the acute services and local community hospitals to facilitate timely discharges, assessing patients at home rather than in hospital; RS also liaises with/works alongside Communities of Practice, Gp surgeries, social workers and local charity organisations.

As a HCA you will be supporting the clinicians to manage the care of these patients by following care plans set as a result of the assessment, and providing support to patients, including personal care and rehabilitation.

The Team uses electronic patient records and individual laptops for mobile working to aid patient facing time, so general IT knowledge is essential.
