Support Worker

An exciting opportunity to join the Rapid Response South team has arisen. The Rapid Response South Devon service has been fully operational in the Southern area since July 2011. The service covers all of Southern Devon area, travel through the whole area will be required. Rapid Response is available to Adults in this area, for up to seven days, who are undergoing an acute physical health crisis, where with appropriate support and services it is possible to maintain them in their own home environment. The service also supports discharges from hospital, particularly for patients at end of life.

The service currently has a 24 hours vacant Support Worker post across the Southern Devon area which we are looking to recruit to.

We are looking for a Part Time person to become part of our team of Rapid Response Staff. You will be experienced in care with either a health or a social care background. It is beneficial to have IT skills as some case recording and use of email is essential.

You will join a dynamic multi-disciplinary team and work with other health professionals to provide care and clinical support to enable people to stay in their own homes during a health crisis. Support and Line Management will be provided by the service Team Leader.

 

Successful applicants will be required to work on a 7 day rota system working both early and late shifts.

Rapid Response Support Workers are Lone Workers within the community, although support is always available via Team Leaders and Out of Hours service.

A full driving licence and access to a car is essential.
