What's the role?

The Associate Practitioner – District Nursing role provides a professional development opportunity for clinicians who demonstrate the capability and commitment to progress to be a Community Specialist Practitioner in District Nursing. You will undertake a period of work based leadership development in preparation for an application to undertake the current post-graduate Community Specialist Practitioner (CSP) Programme or the post-graduate apprenticeship route.

You will be responsible for the holistic management of a caseload of patients as well as the leadership and supervisory responsibility for community nursing colleagues within an Integrated Community Care Team (ICCT) providing adult community nursing services across a Primary Care Network (PCN). This will be progressed under the indirect supervision of a District Nurse.

You will work to professional and regulatory body codes, standards and guidance at all times ensuring your practice is grounded in evidence based theoretical and clinical knowledge, as well as upholding the vision and values of the organisation working within the CQC Fundamental Standards.

What will we offer you?

A minimum 12 month work based development programme designed to enable you to develop the leadership, supervisory and case management skills to secure a place on the CSP/Post Graduate Apprenticeship programme.

During the period of the CSP/Post Graduate Apprenticeship you will have full support from a Community Practice Teacher/Practice Assessor

A guaranteed position as a Community Specialist Practitioner - District Nurse subject to satisfactory completion of the programme and award of SPQ.

Who are you?

You are a registered nurse who has had significant experience of and is currently working within a community nursing environment. You have a passion for community nursing and providing the highest standard of care and are working to embed good working practices through the application of effective leadership skills within your team. You will have the drive and desire to develop professionally as a Community Specialist Practitioner in District Nursing.

You'll need to be resilient, motivated and adaptable to working in an ever-changing environment.

To experience the full benefits of the development programme you will be required to work a minimum of 30 hours per week over 4 days or 37.5 over 5 days, and be prepared to work in a nominated Integrated Community Care Team.

Education/ job requirements and qualifications needed:-.

  • · Current NMC Registered Nurse (adult)
  • · Relevant honours degree at a minimum of 2:2 or completion of HEI bridging programme (e.g. Leeds Beckett PPD module)
  • · Evidence of continuous learning and professional development (accredited or unaccredited gained within the last 5 years)
