Hattersley Group Practice is a friendly busy practice serving over 5600 patients across the Hyde community for over 25 years. The practice is keen to develop and support staff to excel their career aspirations whilst making a positive difference to patients and the community.

The clinical team is constantly developing and is currently made up of 2 GP partners, 2 Assistant Practitioners, 2 Practice Nurses, Mental health nurse, Healthcare Assistant, phlebotomist, acute visiting service and a clinical lead providing clinical support. In addition we have a supportive, welcoming well established reception/administrative team and practice manager. Other GTD support functions such as HR, payroll and governance are also on hand to support as part of the wider organisation. We collaboratively work well with our Primary Care Network to provide additional services such as Child & young adult councillor based at the practice, MSK services, social prescribing, polypharmacy services and complex care nurses. Over the last 2 years, the practice has embarked upon new and exciting projects with our patient health champions such as creating our own practice garden, creating a Tameside wide offer for Parkinsons & dementia groups so it would be a great time to join us to get involved!

An opportunity has arisen for a Prescribing Practice Pharmacist (Senior Clinical Practitioner) to join our practice team to have a pro-active approach to carrying out routine and often complex medication reviews, medicines management, prescription discharges/issues, docman administration and conduct audits for quality improvement initiatives.

The successful applicant needs to be well organised, able to lead and take accountability for individual projects set by GP’s, practice manager or clinical lead, work collaboratively with GP’s, ACP’s, Nurses and polypharmacy to meet the needs of the patient, manage workload of medicines management, mentor other staff members, achieve practice key performance indicators and targets, e.g. QOF, support the delivery of policies and procedures. Take responsibility for own development learning and performance including participating in clinical supervision, acting as a positive role model and undertake learning via our SCP framework as appropriate.

Previous general practice, emis and docman experience is desirable and prescribing is essential. Applicants must be registered with professional body. The hours are 16- 20 hours per week which can worked flexibly and from home however full time hours considered across a portfolio role of gtd services including Advanced Clinical Practice development.
