Purpose of the Role

In this rotational post, the post holder will be responsible for the autonomous clinical assessment, and/or referral or giving advice as appropriate to first contact clients across a wide spectrum of clinical conditions within the services provided by DHU Health Care (LLR). This includes the Emergency Department at the Leicester Royal Infirmary and the Clinical Navigation Hub at Fosse House. This post will involve triaging face to face, and via telephone, and where appropriate signposting to other services, and providing self-care advice.

A commitment to a quality service, excellence in clinical practice and continuous professional development are integral to the role and the development of the service.

The post holder will participate in promoting and advancing DHU Health Care CIC.

Key Areas of Responsibility & Accountability

Clinical

  • Receive referrals from NHS 111 and healthcare professionals.
  • Undertake a comprehensive telephone based clinical assessment without reference to clinical decision support systems.
  • Use clinical knowledge and advanced communication skills to provide high quality advice on self-care as appropriate though the effective communication of evidence-based health care advice.
  • Facilitate the patient pathway through primary, secondary, community and tertiary care. Where possible assisting patients in the appropriate use of primary care and emergency care services and other healthcare related organisations.
  • Manage all calls in a controlled and professional manner treating all patients with compassion, empathy and respect.
  • To practice without direct supervision but support available when required.
  • To assess, plan, treat and refer patients requiring the clinical service according to LRI front Door service Model and area allocated to work i.e. Assessment Zone or Primary Care Stream.
  • Seek advice of specialists as appropriate and follow this advice as appropriate.
  • Managing patients within specified time parameters (this will be communicated separately).
  • To exhibit excellent communication skills when dealing with patients and carers. Non-verbal and verbal skills required.
  • To have the ability to make clear decisions with confidence and communicate these effectively.
  • To provide medications under patient group directives.
  • To work within the parameters of ones skills and knowledge, to recognise the limitations to ones own practice and to work within the bounds of ones training and capability according to the NMC and HCPC Code of Conduct.
  • To recognise the need for, initiate and where appropriate interpret relevant diagnostic investigations.
  • To maintain an awareness of developments in clinical practice.
  • History taking and physical examination of patients.
  • Discharge or refer patients as appropriate to internal or external clinical pathways.
  • To provide health promotion/accident prevention for patients and carers.
  • The ordering, storage, administration and disposal of medicines in a safe, secure manner in accordance with current legislation.
  • To work effectively as part of a multi professional team in collaboration with other members of the service.
  • To share in the commitment DHU Health Care CIC has made to safeguarding and promoting the welfare of Adults, Children and Young People.
  • To maintain appropriate IP&C as per DHU, UHL and national policy.

Person Specification

The job holder will have;

Qualifications

  • NMC/ HCPC Registered nurse/ paramedic.
    • Recognised Minor Illness, Injuries qualification and experience.

Experience

  • Autonomous practice working and multi-professional team working.
  • Evidence of using physical examination and assessment skills in practice.
  • Can demonstrate a broad and sound knowledge.
  • Experience of prescribing medicines under patient group directives.
  • Experience in an Urgent Care setting either in A&E/WIC/UCC/First responder or similar.
  • Experience working within a general adult care setting
  • Evidence of CPD.
  • Experience with Adastra and SystmOne are desirable but not essential.
