Devon Partnership Trust Social Care Teams are looking to recruit 3.75 (FTE) Social Care Assessors to work alongside our established Mental Health Social Care Teams.

These temporary posts (six months fixed term) could be offered as a secondment to suitable DCC candidates (with line manager approval).

These positions are available in the following areas:
Exeter and East Devon 1.75 FTE posts (flexible base across the area)
North and Mid Devon 1 Full Time post (flexible base across the area)
South and West Devon 1 Full Time post (flexible base across the area)

You will be working in our exciting mental health social care team to offer advice and complete assessments for adults of working age who are open to secondary mental health services; the role will involve both telephone and community assessments. You will be supporting people to retain their independence, taking a variety of contacts and referrals from the public and other professionals, providing advice and information, undertaking holistic assessments and support planning, working in partnership with other agencies, making onward referrals, and reviewing services. You will work in partnership with our front line Community Mental Health teams.

We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a social care or health environment and have a good understanding of human needs. You will also ideally have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team. Previous experience in a mental health service is preferable, but not essential as training and supervision will be offered as part of the role.

You should be an effective communicator both verbally and through written formats, able to prioritise work load, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people.

This role requires the ability to fulfil all spoken aspects of the role with confidence and fluency in English.
