Are you a qualified District Nurse with at least two year experience, looking to use your clinical skills & knowledge to make a difference to patients?

We are looking for Senior District Nurses who have worked at a senior level and have additional skills including non-medical prescribing. However, those currently studying for non-medical prescribing will be considered. Candidates should be working at masters level, and bring expertise such as enhanced physical assessment skills and recognised modules at level 7.

The role will support the management of our most complex patients, and provide clinical leadership for our teams, especially Long Term Conditions, Wound or End of Life care.

We are changing the skill mix of our current teams in this area, to allow clinicians to provide continuity for patients, by working together with the patient to plan their individual care needs for good outcomes.

We are supporting a team ethos approach for working together, and in response to the recognition of the increasing demand of complex patient needs, employing those with the specialist training, experience and advanced skills.

Adding this significant clinical expertise will drive the quality of our work, and produce excellent patient outcomes. As the senior nurse, this role will drive the clinical leadership for teams, whilst promoting high standards of assessment for personalized care plans, enabling excellent care delivery to patients, in their own home or within local clinics.

We are looking for people who think a bit differently - to be part of a nurse-led NHS team that is self-directed, can share responsibility and take decisions together.

You’ll need to be bold, brave, creative, and willing to take responsibility.

We have a full time post, 30 hours & 22.5 hours at Foster Street in Maidstone.

This means that we reserve the right to change the hours detailed in this advert to full time or part-time to meet the service requirements. Please note that the salary stated is based on full time working and will be pro rata for part time working.

Agenda for Change posts based within the Dartford, Gravesham & Swanley areas also receive 5% high cost area allowance on top of basic salary, subject to a minimum payment of £1,017 and a maximum of £1,762. Please note that this element does not apply to bank positions.
