Community Staff Nurse - Band 5 - Sudbury Community Health Team - 37.5 Hours per week - Permanent & 30 & 37.5 Hours per week Fixed Term Contract

Could you help West Suffolk Hospital to deliver the best quality and safest care to our local community?

Some exciting opportunities for Community Staff Nurses, looking for a new challenge, have arisen in the Sudbury Community Health Team. The team are based in the new Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Community Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way. The successful applicant will join a self-motivated, friendly team of community nurses and therapists. They will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi disciplinary team including the Community Matrons and Interface Geriatrician.

The successful candidate will be assessing the patients by carrying out home visits and planning and implementing a patient’s individualised care. On site at the health centre you have access to a range of treatment facilities and may be involved in delivering nursing care in specialist clinics. They will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting.

Experience of palliative care, wound management and catheter management will be an advantage. You will have the chance to be fully involved in working with the MDT and integrating with ACS, hospice and hospital staff and will need to be committed to ongoing learning and development in the team.

This will be the perfect opportunity if you would like a role where you can use a broad range of nursing skills, work independently but with the support of a close knit team and work with patients in their own home environment to improve their health. A one year fixed term post is also available for maternity leave cover and a secondment would be considered to give experience of community nursing.

The successful candidate for this post will be required to apply for a Disclosure & Barring Service (DBS) Disclosure at Enhanced level.


For Band 1 and Band 2 posts, the cost of the DBS Disclosure will be met by the Trust. However, any employees at Band 1 and Band 2 who leave the Trust within six months of commencing their post will have a £10 administration fee deducted from their final salary. For posts at Band 3 and above, the cost of the Disclosure will be met by the appointed employee and deducted in instalments from their salary over a period of three months. If the employee leaves the Trust within three months of commencing their post, any outstanding payments will be deducted from their final pay.
