The Cissbury Integrated Care Primary Care Network (PCN) Community team have a vacancy for a Proactive Care Coordinator to cover a year's maternity leave. The role involves coordinating care for patients by working with a multidisciplinary team. This involves discussing the patients' needs with patients, families and carers, along with health and care professionals, to enable patients to remain in the community and prevent unnecessary hospital admissions. The Proactive Care Coordinator role is pivotal in ensuring all our patients receive the best possible care.

This role will support health and social care professionals in coordinating all key activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred.

The successful candidate will need to have a good understanding of health and social care services, be an excellent communicator, and be able to establish efficient monitoring and quality assurance systems.
