An exciting opportunity has arisen for an experienced, innovative, enthusiastic, forward thinking Band 7 Diabetes Specialist Nurse with exceptional diabetes nursing and leadership skills to join the Lincolnshire Countywide Diabetes Team. This role provides an excellent platform for a senior diabetes nurse who is focused on building upon and continually improving quality diabetes care, safety and the patient experience.

Are you passionate about providing high quality care to your patients with diabetes, their families and carers and teaching and supporting GPs and community health colleagues about diabetes care and management?

We are looking for an experienced Diabetes Specialist Nurse to join our Diabetes Team. The team works as part of a multidisciplinary team, with DSN, Dietitians, Consultants, GPwSI, Podiatrists and Admin. This is an exciting opportunity to join our forward thinking and dynamic team where you will be supported and encouraged to be a part of our ongoing service development and dedication to making a difference to patients with diabetes.

We provide a comprehensive service including:

  • Nurse Led Clinics and caseload management
  • Complex Type 2 diabetes management
  • Structured Education for Type 2 diabetes
  • Health care professional Education


  • Monthly Departmental meetings
  • Access to medical staff
  • Study leave appropriate to the role

We are looking for a self motivated, team player, forward thinking experienced nurse who is enthusiastic about diabetes care.

Do you have the following to join our team:

  • To be a registered nurse
  • Relevant post registration training (in diabetes, teaching & learning)
  • Independent nurse prescriber or able to demonstrate an ability that will enable attendance of a prescribing course within 6 months.
  • Significant experience of assessing and providing evidence based care to people with diabetes
  • experience in the management of community based diabetes emergencies to avoid hospital admission

The role carries educational responsibilities and there will also be opportunities in leadership, management & development within specialist aspects of the service.

The Community Diabetes Service is an enthusiastic and forward thinking team delivering high quality specialised care to adults living with diabetes in Lincolnshire. We provide evidence based diabetes care which is able to meet the complex needs of individuals accompanied by the support of a skilled MDT that includes dietitians, GPs, Consultants Neighbourhood Teams and specialist nurses from other disciplines.

This post is for 37.5hrs and will be based in Skegness area (base to be confirmed) to support the patients living in the North East of the county. The successful candidate will have excellent interpersonal and communication skills and be able to prioritise tasks. The ability to manage time effectively and work flexibly within the team is essential and you will need to be able to commute countywide at times to meet the needs of this role. You must have access to vehicle to be able to travel across the county, including rural areas.
