Gloucestershire Health and Care NHS Foundation Trust was formed in October 2019, following the merger of 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust. The two Trusts joined together to provide integrated mental health, physical health and learning disability services to communities.


JOB TITLE:Occupational Therapist, Care Home Support Team

Band: 6

Salary: £31,365 - £37,890 per annum

Hours: 37.5 hours substantive

The Gloucestershire Health and Care NHS Foundation Trust, Care Home Support Team is a multi-disciplinary team made up of registered nurses, an occupational therapist and a dietitian. The extended team also consists of a designated CCG pharmacist and registered mental health nurses. The team all work together with managers and staff of residential and nursing homes across the county to build relationships, promote best practice in care, and provide training in order to support providers to deliver safe high quality care.

This highly respected and effective team has done much to support the care home community in their continual drive to raise standards of care and to act as a link between care homes and other stakeholders in the wider health and social care arena in Gloucestershire.

An exciting opportunity has arisen for an enthusiastic, self-motivated individual to join the Trusts’ Care Home Support Team as an Occupational Therapist, you will be required to contribute to the smooth running of the team and support all members. You will require good IT skills and the ability to work with people at all levels, with good communication skills, both verbal and written.

The successful candidate will be a reflective practitioner, able to demonstrate effective clinical reasoning skills, be able to apply theory to practice, demonstrate effective leadership skills and demonstrate effective and sensitive communication with Residents, Care staff , Home Managers and Colleagues across the system .

You should have the ability to use your own initiative but also be team orientated and have a flexible approach to work. You will also require skills to develop and present training packages to care home staff.

 

We are looking for someone who is dedicated to help the team deliver and promote the highest levels of quality person centred care, together with the aptitude to work autonomously using your own initiative, with excellent prioritisation skills.

For positions Band 8a and above there is a requirement to participate in an on-call rota to ensure a high quality of care for patients at all times and continued support to all staff with access to the right knowledge and expertise.
