Do you have experience in the role of continuing care/facilitating an effective discharge of long term patients and improving discharge planning?

Anopportunity has arisen to become theContinuingCare Co-ordinator withinPaediatric Critical Care here at the Children’s Hospital.

The successful applicant will be expected to lead the discharge process of children with complex health needs/long term ventilatory needs into the community. Work as a role model for all staff and being an expert in communication with families and the multidisciplinary team is essential. Along with working alongside the Trust's Complex Care Team.

Experience in other aspects of Nurse Management would ideally include Sickness Management, PDR, Conflict Management and Resolution and performing interviews.

Sheffield Children’s NHS Foundation trust hosts Embrace, the only Paediatric/ Neonatal transport service within the British Isles and PCCU work closely with this team.

This is customarily payable over 4 months as a salary deduction once employed by the Trust. If you receive a conditional offer letter, complete your DBS application and later withdraw from the recruitment process, an invoice will be raised via the Trust's finance department to recoup the outlay.
