Rapid Diagnostic Service Care Navigator

Warrington and Halton Hospitals has introduced a fixed term post within the Cancer Services team in order to support theRapid Diagnostic Service. This is a very exciting 12 month project and an opportunity for us to be innovative in the way patients are managed through their diagnostic journey.

This is fixed term/ secondment post - 12 months from start date.

Throughout the project the RDS Navigator will be:

  • The main point of contact for patients to Navigate them through the RDS pathways
  • Will be central to effective communication between health care professionals both within the Trust, GPs and Commissioners
  • Ensure all data requirements are collected and input onto the relevant database
  • Self-driven, highly organised and have excellent communication skills
  • Work alongside the RDS Specialist Nurse to provide a robust patient journey
  • Will work with existing teams to reduce confusion and problem solve ensuring no delays in patient pathway
  • Will improve the patients experience by ensuring that the diagnostic pathway is proactively planned and organised, to ensure that cancer waiting times standards are achieved and, crucially, that patients receive their treatment in a timely and efficient way.

The role will involve liaising with patients, their relatives and clinical and administrative teams, both within the Trust and primary care to coordinate appointments, investigations and treatments. The post holder will also provide information about patient pathways to support the achievement of waiting times standards and ensure effective shared learning from any pathway breaches. The data collected will form the Trust’s mandatory submissions to all national and statutory requirements, for example - Cancer Outcomes and Services Dataset (COSD), and Cancer Wait Times database. Therefore experience of data inputting, clinical IT systems and an understanding of patient pathways and tracking is essential.

Depending on the classification of the post, in-line with DBS guidelines, a Standard or Enhanced disclosure will be required which may include a check of the ‘Vulnerable Adults List’, the ‘Children’s List’ and/or the Independent Safeguarding Authorities (ISA’s) barred lists.

A charge of £60 (Enhanced) will be made by the successful candidate which will be deducted from the first 3 months’ salary. In addition to this, you will also be required to re-pay any future costs that may arise in relation to DBS fees.
