Leicestershire Partnership NHS Trust (LPT) provides a range of community health, mental health and learning disability services for people of all ages. Delivered through over 100 settings from inpatient wards to out in the community, our 6,500 staff serves over 1m people living in Leicester, Leicestershire and Rutland.


We are looking to appoint a highly skilled LD Practitioner / Clinical Case Lead, who will be central to the delivery of meeting the LLR Transforming Care Agenda.

This is a critical and high profile agenda, supporting and assessing patients with a Learning Disability and or Autism, who are in hospital settings in LLR and OOA placements / hospitals who are close to, or ready for discharge, to ensure that the appropriate care packages are in place to support successful community placements.

The successful candidate will possess specialist skills and knowledge in their areas of responsibility, which will allow them to lead this programme and others, developing processes and procedures to ensure that timescales and objectives are met in line with the TCP agenda to reduce the unnecessary use of inpatient services when meeting the needs of service users from Leicester, Leicestershire and Rutland.

The right candidate will be self-motivated and confident, working across organisational boundaries and with staff at all levels, to form positive relationships with the partners across our CCG, LA and provider network and create integrated (partnership) solutions.

You will require a relentless focus on needs of service users, excellent communication skills, energetic leadership and ambition for our services to get the best out of others. You will have a proven track record of collaborating, influencing and empowering to achieve success. You will require diplomatic skill whilst being assertive and willing to challenge at all levels.
