An opportunity has arisen for a Band 5 Nurse Assessor to join the Responsive Services team, based in Horsham and covering Horsham, Crawley and Mid Sussex.

The role involves working in the community in a multidisciplinary team, providing an admission avoidance service and support to patients upon discharge from our hospital through our new Home First Programme. The post holder will need to work autonomously as well as part of a team. They will be required to work shifts that cover 8am-4pm or 12pm-8pm, up to seven days a week, including one in three weekends. Though based in Horsham, they will be expected to work in the other areas if the need arises.

You will be required to demonstrate excellent communication skills, experience of how to help people manage their health conditions in the community setting, and knowledge and practical skills such as wound care, venepuncture, management of the End of Life pathway, and management of the deteriorating patient. Development of these skills will be supported.

We are a short term service that has a key focus on facilitating discharges and reducing admissions to hospital, and we work closely with our medical colleagues including GPs and community elderly care consultants, providing a joint approach to the management of our patients. We are a rehabilitation and reablement team, focusing on supporting patients to meet their personal goals.

We provide expert interventions and advice to our patients and the local community on managing illness and exacerbations that could result in the deterioration of health and wellbeing. We see people in the community in their own homes.

We offer a well-supported working environment, including policies to support and ensure good practice, and there are opportunities for professional and clinical development. This role also includes a leadership role within the team through clinical leadership, caseload management, and supervision of other staff.
