We are looking for a dynamic and enthusiastic Community Sister to join our Burgess Hill Primary Care Network (PCN) team based out of Hurstpierpoint Health Centre.

The emerging PCNs are groups of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated and integrated health and social care.

The Community Sister has overall responsibility for a designated caseload and will manage care delivery by the integrated team. The post holder will attend health and social care meetings to discuss complex patients at risk of admission and devise contingency plans. They will also be responsible for staff management including personal development reviews, supervision and staff development. They will be an experienced community nurse with a wide range of clinical competencies and a mentorship qualification. Clinical support will be provided by the Clinical Leads.
