An exciting opportunity has arisen to work in our multi-professional community team, based in Hurstpierpoint and covering Burgess Hill and its surrounding villages. We are proud of the quality holistic care we provide, focusing on self-management with patients using a proactive care approach. The team is multidisciplinary, comprising of health, social, mental health practitioners and third sector staff.

This post is ideal for somebody who would like the opportunity to experience community work or develop their interests in the community setting further.

The role incorporates the day to day management of patients within the community under the supervision of the Case Manager and Clinical Lead, involving a variety of skills including wound care, continence management, palliative care, health promotion, venepuncture and more. Our Community Staff Nurses benefit from having continual opportunities to develop an excellent skill set and become autonomous practitioners, whilst working as part of a team delivering high quality, holistic care to patients in their own homes. Training will be available and the post holder will be supported with an induction and a personal development plan. Prior experience in a community setting is desirable, but a full induction and training will be provided. We welcome applications from newly qualified nurses, who will be fully supported with preceptorship.

You must be able to work autonomously as well as part of a team. The team use SystmOne mobile working.

You must hold a full driving licence and have the use of a car for work purposes.
