South Southwark Social Prescribing Link Worker

Are you passionate about making a difference in Southwark?

We are looking for an enthusiastic social prescriber/link worker to join the South Southwark Primary Care Network. This is an excellent opportunity to work directly with people with more complex needs and help them to access the support they need to live independent and healthier lives. This is especially important during these times when people will need this support more than ever.

As a Social Prescriber link worker, you will provide time-limited support to local residents, understanding their needs and helping them to access the support they need to achieve their goals. This will involve building links and liaising with various services/staff in the local area – you would work closely with general practices, health, social care, voluntary and community organisations.

This is a fantastic opportunity to become part of a network that will develop and improve patient care. You will work with local residents, services, and contribute to how we shape this service to make it work for our local residents.

The successful candidate will work across different areas within our network and be embedded within general practice. They will exercise a flexible and proactive approach and work well as part of a multi-disciplinary team.

This is an exciting role which will develop and grow with time and the successful candidate.
