Temporary Full time 37.5 hours per week - Bridlington 

This is an exciting opportunity for a reliable, flexible and enthusiastic experienced senior community nurse who demonstrates a commitment to provide high standards of care to patients and possesses strong clinical leadership qualities, to take on role of Case Manager within the Community Nursing Team based in the  Driffield or Bridlington Neighborhood Care Services (NCS). Appointment to the post will be on a fixed term basis to backfill for Maternity leave/study leave.

The successful applicant will either hold the Specialist Practitioner Qualification in District Nursing or equivalent, or be a suitably experienced senior community nurse. Effective caseload management is key to this role. The post holder will therefore be responsible for your own caseload, working closely the your Clinical Team Leader and other senior members of the team to lead and manage others, particularly members of their caseload team, which will include staff nurses, nursing associates and health care support workers.

The successful applicant will need to have excellent communication skills and have an adaptable approach to work in order to respond to the dynamic changes that occur in community nursing on a day-to-day basis.

Additionally, you will need to demonstrate IT literacy and this post requires that you have the ability to travel between locations to carry out your work and to attend training and meetings. Daily access to transport is therefore essential.
