Pharmacy Technician

Overview

A new and exciting opportunity has arisen for a Pharmacy Technician  to join Central & West Warrington Primary Care Network (PCN)

Central & West Warrington PCN is a collaboration of 5 GP Practices (Causeway Medical Centre, Dallam Lane Medical Centre, Eric Moore Partnership Medical Practice, Folly Lane Medical Centre and Penketh Medical Centre) with a total registered population of © 45,000 patients, within Warrington, Cheshire.

The candidate would support the practice team to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes

Post-Holder

Main duties for the Post

The position will be through employment with a lead Practice for the PCN, where you will be integrated within the  multi-disciplinary primary care team to work across the 5 GP Practices that form the CWW PCN to deliver the healthcare services as part of the Network

To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

Support the Practice teams by helping with queries relating to prescription requests and looks for ways to improve the efficiency and safety of the prescription issuing process, ensuring queries are resolved and scripts issues in line with the timeframe in the practice procedure.

Review out-patient letters which include changes to drug treatment and make changes to patients repeat medication list as appropriate.

Working with the Prescribing Lead GPs, PCN Clinical Pharmacist and other members of the team to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures

To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

Respond to queries from GPs and other clinical staff and contact patients where necessary.
