 Are you passionate about delivering high standards of nursing, with an interest in palliative and end of life care?

We are looking to recruit enthusiastic Registered Nurses who want to make a difference to join our established Community Team. Working in the local community, this role will give you the opportunity to deliver individualised holistic evidence-based nursing care to patients and those important to them

Main duties will include:

· Supporting those who wish to die in their own homes by undertaking urgent and routine assessment visits to those facing the end of their life

· Coordinate and plan care utilising a range of services, such as, Hospice at Home, District Nursing and Marie Curie etc.

· Working collaboratively with patients and their families, and other health care professionals to provide shared patient care

· Undertaking symptom control and personal care, and provide immediate support to the patient and their family

· Recognise signs of dying to enable discussion with patients and families regarding choices at end of life, to initiate anticipatory prescribing so that symptoms can be addressed in a timely way

You will be required to demonstrate your abilities in the following areas:

· First level Registered General Nurse

· Ability to assess, provide and evaluate high standards of care without supervision

· Be organised and have strong decision-making skills

· Ability to supervise unqualified staff

· Understanding of the issues affecting patients and families at the end of life.

· Good holistic assessment skills and symptom management knowledge. However, applicants will be supported with their own learning and development

· Capability to work remotely and flexibly

You will be required to work a variable shift pattern between the hours of 08.00 and 20.00 over 7 days. You will receive an unsocial hour’s enhancement for weekend and bank holiday working. All candidates must have a full clean driving license and have access to a car.
