North Region - The position for this region covers Betsi Cadwaladr Health Board with hub offices located in the region close to acute care providers

An exciting opportunity has arisen for full and part time Medical Examiner Officers to join a new all Wales Medical Examiner Service as part of the national implementation of the Medical Examiner system in England and Wales. The aim of this service is to be able to scrutinise deaths within acute hospitals within one year of development of the service, and by April 2021 to be able to take on the responsibility of reviewing the circumstances of all community deaths in the corresponding areas. To do this, we aim to develop a responsive and fluid service, which is adaptable to the needs of the Health Boards, our patients and our workforce.

This service will:

Strengthen safeguards for the public by: providing robust, systematic and independent scrutiny of all deaths not referred directly to the Coroner (cause of death and circumstances impacting on the death); providing intelligent analysis and system level reporting of concerns found during scrutiny; and ensuring that the right deaths are referred to a Coroner

Improve the quality of death certification by: providing expert advice to doctors (QAP) based on a review of relevant health records

Avoid unnecessary distress for the bereaved by: answering questions about the certified cause of death or care given, or that arise from unexpected delays when registering a death.

Medical Examiners will undertake an independent scrutiny of deaths in order to establish an accurate cause of death, and identify cases where the circumstances surrounding that death requires further investigation. The role of the Medical Examiner Officer is to support Medical Examiners in their role in scrutinising the circumstances and causes of death. This support will include:

Case discussion and assisting in the formulation of causes of death with the attending medical practitioners and Medical Examiners.

Discussion with the bereaved relating to explanation of causes of death and identifying, and gathering information relating to concerns.

Relaying the Medical Examiners thoughts and advice to medical professionals, Coroner’s offices, Registration services and the bereaved.

To be a point of contact and source of advice for relatives of deceased patients, healthcare professionals and coroner and registration services about the Medical Examiner function.

This role will incorporate administrative skills in ensuring necessary paperwork is accurate and complete, but also excellent communication skills during liaison with doctors, nurses, families, and where appropriate, the Coroners' and Registration services. Part of the day to day work will also involve the review of patient notes and gathering information the Medical Examiner may require to undertake scrutiny of the medical records, so a good working knowledge of medical terminology and clinical understanding is necessary.

The Medical Examiner Officer role is anticipated to be hugely rewarding and will improve personal skills for the successful applicant. It is also a highly responsible, challenging and involved role so will require a dedicated and driven applicant.

NHS Wales Shared Services Partnership is seeking applications from professionally registered clinical staff, or from individuals who have significant experience in mortality review, patient safety or bereavement care. Applicants should also be naturally curious, empathic, be able to demonstrate attention to detail and be committed assisting with the local development of the Medical Examiner Service.

Applicants must have undertaken the mandatory Royal College of Pathologists’ Medical Examiner e-learning modules. Once in post Medical Examiner Officers will undergo formal training appropriate to the role, as provided by the Royal College of Pathologists and via completion of a training programme portfolio and competence assessment, which must be completed within 12 months of starting in post. The role requires continuous training as part of a lifelong learning role and continuous professional development.

Initial contracts will be for a fixed term of 12 months, or shorter, which will be dependent on the successful completion of work-based training. Secondment options are also available and will be considered but support from your current employer would be essential.

You will be required to work in partnership with the Medical Examiners, to assist with the development of the service from start-up and regularly contribute to review and refining this over time. Local Medical Examiner Offices will be based largely on District General Hospital sites, but the role will involve multi-site working within the Medical Examiner Region or between Health Boards, dependant on the location of cases on any given day or week.
