Durham Dales Health Federation is a formal collaborative of the 12 General Practices in Durham Dales that have agreed to work together to co-ordinate and provide some elements of patient care jointly across the whole area.

We are looking for an experienced and motivated Team Lead to support our Clinical Services Manager and Emergency Care Practitioners/Nurse Practitioners. 

Within the role you will provide care to a range of patients within your scope of practice. Working within this team, you would have the opportunity to work alongside GP Practices, care homes and community hospitals within the Durham Dales locality.

Providing advanced levels of autonomous practice by assessing, investigating, diagnosing, treating and referring patients acting in accordance with relevant professional codes of conduct.

Ideal candidates will have a range of experience gained working in a GP practice, Community or urgent care setting (primary care, minor injuries, accident and emergency or walk in centre), have qualifications and registration to support their clinical practice.

Excellent IT and communication skills are a key requirement of the role as well as a flexible approach.

Independent non-medical prescribing and Clinical Skills qualifications essential as well as a current UK Driving Licence.

Salary equivalent to Agenda for Change Band 8a. Mileage paid, enhancements for weekend working and NHS pension available.
