We are looking for motivated, enthusiastic individual with appropriate experience to join a dynamic multi-disciplinary team within Drugs and Alcohol Team at offender healthcare. CHCP CIC currently provides integrated offender healthcare services across HMP Hull and HMP Humber. We deliver a community-based model, bringing clinical expertise to the patients in our care, working closely with the multi-agency workforce.

The successful applicant will be part of a multi-disciplinary, multi-agency team, working in partnership with service users, carers and other agencies to support individuals in their long term recovery from substance misuse within HMP Hull. You will work within a supportive multidisciplinary team providing a range of recovery focused interventions across offender healthcare.

 

Your role will be to:

  • Have a good understanding of substance misuse, including alcohol

  • Knowledge of barriers to recovery and thoughts on how to engage service users

  • Facilitate assessments and support a range of evidence based clinical interventions to service users in line with national guidance

  • Support service users to address their holistic needs and on-going recovery from substance misuse through facilitating a person centred care plan of need.

  • Undertaking recovery based psychosocial interventions with patients; inclusive of one to one work and group work facilitation

  • Coordinating through the gate care with community services.

  • Facilitate nursing interventions including second checking of controlled medications

  • Work closely with partner agencies including statutory and non-statutory services

  • Manage a case-load of service users who may be subject to safeguarding children / adult proceedings, have concurrent mental health issues, physical health in addition to their substance misuse.

  • Work within an extremely varied role
