We are seeking to recruit a Senior Diabetes Specialist Nurse to work within our community diabetes service for Brighton, Hove, Lewes, High Weald and the Havens in East Sussex.

This is an exciting opportunity to work within a multidisciplinary team of consultants, nurses, dietitians, podiatrists, psychologists and support staff, and contribute to the development of this innovative service.

The Senior Diabetes Specialist Nurse will work in partnership with patients with type 1 or type 2 diabetes in a community setting, aiming to improve outcomes for patients. This includes the development of clinic-based care, home visits, and patient education programmes for diabetes management.

The post holder will also establish and maintain effective working relationships with health professionals in both primary and secondary care, ensuring the provision of high quality specialist diabetes care to patients and carers at all stages of the care pathway.

This role offers an excellent opportunity to develop clinical and service development skills, as well as experience of working within a multidisciplinary team integrating physical and psychological support.
