The Huntercombe Group is excited to have opened a new satellite service managed by Eldertree Lodge called Oakwood House in Stoke-on-Trent.
Our key objective is to deliver an end to end model of care and housing for all individuals accessing the discharge pathway that takes them from hospital admission through to community in the shortest possible timeframe.
Oakwood House is our exciting community supported housing service for people with a diagnosis of ASD/ADHD and/or Mild/Moderate Intellectual Disability with Communication and Sensory needs.
We are developing a community support worker team initially working into Oakwood but this may develop to cover other areas in Staffordshire in the future.
As a Support Worker you will support and enable individuals to live a fulfilling lifestyle in their own homes within the community that promotes and enhances personal choice, independence and overall wellbeing.
You will support and enable individuals in all aspects of daily living to encourage independence through the use of person centred approaches. This could include various personal care, domestic tasks, meal preparation, prompting and administering medication, budgeting, engaging in activities and accompanying individuals at leisure activities/ other outings in the community.
Hours of work and work pattern: Full and part time hours are available. Shifts are 12.5 hours. You will be required to work days and nights (waking).

Passionate about forming therapeutic relationships with people
Committed to a person-centred approach
Enthusiastic about working as part of a dedicated team
Resilient and able to work under pressure
Previous experience working in a residential or support living setting with individuals with a learning disability or mental health illness would be an advantage but not essential
An enhanced DBS (England) or PVG/Disclosure Scotland (Scotland) Disclosure will be required for all successful applicants. Costs will be met by the Company.
