We are looking for motivated, enthusiastic individual with appropriate experience to join a dynamic multi-disciplinary team within offender healthcare. CHCP CIC currently provides integrated offender healthcare services across HMP Hull and HMP Humber. We deliver a community-based model, bringing clinical expertise to the patients in our care, working closely with the multi-agency workforce.

The post holder will be expected to work as a member of the mental health team within a secure environment, under the supervision of the management structure. The post holder will provide assessment and interventions to patients, from a primary care to a secondary care level including crisis intervention. Working within the care programme approach and liaison with outside agencies to support discharge planning whilst seeking to reduce stigma and increase access to information about mental health and resources available.

The post holder must be highly motivated and be committed to continued professional development that ensures a high quality cost effective care to the client group.

It is expected that the post holder will predominantly work within HMP Humber, and will be expected on occasions to work across sites at HMP Hull. The position will also expect when demand arises to support the generic health team with delivery of health care.
