About the role

An exciting position has arisen in our Herts Valley service. We are looking for an enthusiastic experienced Clinical Nurse specialist to join our multi-disciplinary community Rheumatology team. You will participate in triaging urgent cases to specialist care and manage non-urgent rheumatology patients in the community alongside stable review patients on DMARDs.

We are looking for individuals who are passionate about patient care and the evolving progression of nursing as a profession. They will be nurses with a proven record of continuing education and experience in Rheumatology. They will be autonomous and self-directed but will also be good team players, recognising the strength of a strong team and mutual support. They will be responsible for developing a workforce to ensure the long term success of the service.

Advanced clinical assessment and consultation skills are essential for this role. In addition the successful candidate will be a non-medical prescriber. Excellent communication skills and the ability to work in a variety of settings are essential. Above all else the individual will feel passionately about improving the care and lives of people who suffer from these long term, often unpredictable conditions.’

You’ll maintain a portfolio relevant to the role of a Clinical Nurse Specialist in Rheumatology and will provide expert clinical practice to the service. You’ll play a pivotal role in the integration of research evidence into practice. This will be delivered by enhancing quality in the areas of assessment, diagnosis, management and evaluation of MSK Rheumatology to improve the outcomes for patients.

Our service in Herts Valley

Our Herts Valley service is a fully integrated MSK service that provides a single-point of access for NHS patients enabling a seamless and efficient patient journey. As part of our integrated community pathway we deliver community Physiotherapy, Pain, Podiatry, Rheumatology, Hands, CATS and Postural Stability. You will be part of a large, dynamic multidisciplinary team which includes MSK Physiotherapists, Advanced Physiotherapy Practitioners, Occupational Therapists, Podiatrists, GPs with special interest in Pain and Rheumatology, Consultant Rheumatologists, Psychologists, Hand therapists and Rehabilitation Therapists. The integrated service design allows clinicians to work together to deliver seamless and evidence-based care for patients and enables inter-disciplinary learning.

 

We deliver our services out of a number of different hub site clinics and GP practices across Herts Valley localities. This encompasses Dacorum, Hertsmere, Watford and Three Rivers and St Albans and Harpenden. There is the expectation that you will be flexible to the needs of the service and you may be based out of a couple of different locations within your working week.

We also operate out of a number of community gym settings in the locality and can provide bespoke and supervised rehab programmes within a fully equipped gym environment, allowing patients to transition seamlessly on discharge.
