Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire and Sefton area. The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites.

Central Lancashire are recruiting for Band 6 Mental Health Practitioners and Band 3 Health Care Assistants for a newly formed Frequent Attenders Team.

Service users within the “Frequent Attender Programme” are high intensity/complex individuals with multiple issues and needs. They can be frequent attenders within the Emergency Department or high users of Section 136. Many people will also be receiving care from Home Treatment Team, Personality Disorder Network or CMHT.

We need staff who are able to work assertively and empathically with people within the Locality to encourage their engagement with multi agency services and support them within their recovery. This will involve working collaboratively with other agencies to aid engagement to meet the needs of the service user.

The B6 Mental Health Practitioner’s will provide a comprehensive assessment and develop collaborative care plans with Service Users and other agencies and where necessary signpost to other services within the community. For those Service Users that are already known to mental health services you will work collaboratively with the Care Coordinator to develop a robust plan of care that Multi agencies are involved with to support the person and prevent crisis. If they are unknown you will case manage for a short period of time to enable referrals to the relevant organisation/ agency.
