Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire and Sefton area. The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites.


SPECIALIST COMMUNITY FORENSIC TEAM (LSCFT)

Lancashire and South Cumbria NHS Foundation Trust is a Community and Mental Health Trust whose services include mental health and substance misuse services to the residents of Lancashire and South Cumbria. The Trust also provides low and medium secure care and forensic services.

Exciting opportunity has arisen to join a newly established team supporting forensic service users to achieve a safe and timely transition into the community.

The role will be embedded within a Multi-disciplinary team with a specific focus on expediting the safe, smooth discharge of service users within Guild Lodge to a range of appropriate community placements.

The team will support clients across Lancashire and South Cumbria.

  • Promote service users’ independent living
  • Form positive therapeutic relationships whilst maintaining professional boundaries that support service users’ own decision-making and build on their strengths.
  • Provide support with daily living
  • Help service users to gain access to resources
  • Provide information on health promotion
  • Support service users with involvement/participation in their care plans
  • Support and encourage service users to live full and independent lives
  • Promote social inclusion by enabling service users to engage in mainstream activity in the domains of employment/vocation, education, leisure, social networks and community connections

Guild Lodge hospital site is situated in beautiful rural area, yet close to motorway links from all directions of travel.
