Primary Care Assistant Practitioner, Band 4

An exciting opportunity has arisen for a Primary Care Assistant Practitioner (Band 4) working for Greenwich District Nursing Twi-Light Services.


This is a part time post, 25 hours per week, based in the London Borough of Greenwich.

Are you friendly and flexible? Would you enjoy working with a diverse client caseload delivering nursing care and promoting healthy living? We are looking for someone with excellent communication and team working skills with the ability and willingness to learn and develop. It is essential that you have the Primary Assistant Care Practitioner course completed at foundation degree level or nearing completion.

You will work both on your own and with nurses in the team, visiting patients in their own home; you would be expected to feedback to the team leader during regular handovers. You will have experience of the use of various IT applications including Microsoft and Rio, (Rio training can be provided). Knowledge of personal health and safety responsibilities is essential.

You should be a committed, highly-motivated and forward-thinking individual who should be able to work autonomously, and as part of a team. You must possess excellent interpersonal and communication skills, a flexible approach and be able to demonstrate your ability to prioritise and organise your work

Adult Community Services Greenwich is part of Oxleas Foundation Trust, and you will have regular supervision and access to training opportunities.

Our dedicated teams respond to urgent calls in different parts of the borough and provide a high standard of individualised care for patients in their own homes and clinics.

Due to the nature of the role, a full UK driver’s license and access to a car for work purposes is essential.
