£30,401 to 37,267 per annum

The West Wales Regional Partnership Board developed a regional response to the Welsh Government’s ‘A Healthier Wales’ programme to support transformation activity across the region and at local level linked to the Area Plan and to Hywel Dda University Health Board’s Health and Care Strategy.

One of the programmes funded was Creating Connections for All. This programme will focus on the development of a regional set of activities to incentivise active citizenship. It will be based on an intergenerational approach that fosters and nurtures connections to alleviate loneliness and isolation prevalent in many of our communities.

The Community Connector Plus team is being established to deliver two key elements of this programme:

1. To review and reinforce the vital role of system navigators and community workers operating within the region and ensure a consistent and coherent approach moving forward, with a more sustainable funding model to support this role.

2. To roll out a consistent model of age, dementia and disability friendly communities that builds on existing local initiatives, branded as West Wales is Kind. This will involve recruiting Community Champions and working with key people within communities to embed the West Wales is Kind framework.

The Community Connector Plus Regional Coordinator role will lead the design and delivery of this programme of work, and manage the team of Community Connector Plus officers across Hywel Dda.

This post is offered on a fixed term / secondment basis.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply
