Advanced Clinical Practitioner - Primary Care

We have various positions available in GP Practices in Manchester, Oldham and Tameside and Glossop. All of our clinicians are provided with CPD/Leadership hours within their roles as well as training opportunities depending on service needs.

You will:
• Be a team player working closely with the practice team whilst able to work autonomously to make clinical decisions for the patients in your care
• Have the ability to fully assess/manage and treat patients of all ages
• Be supervising and supporting trainee practitioners
• Be supporting junior members of staff if required
• Have knowledge and experience of Primary care policies process and achieving Quality indicators
• Have experience with patients with complex needs/ polypharmacy and chronic conditions
• Be able to liaise and work with other members of locality/neighbourhoods and multidisciplinary teams within CCGs

Most importantly you will be fully supported by the Practice manager, GPs and a Primary care clinical lead and Head of Locality as well as the gtd wider infrastructure including clinical governance and training opportunities within our academy.
There will be scope explore/develop a portfolio role within the company if desired (services can be seen in the summary below).

The role incudes a generous CPD entitlement to ensure development in practice and how you use this time is flexible and negotiable with your clinical lead and manager.
You will be involved in Practice meetings and have the ability to influence change and improve care within diverse patient populations as well as the opportunity to lead on specific clinical projects within the practice team..

The post holder will hold either an MSC in Advanced Practice or be working towards RCN credentialing accreditation and within your professional boundaries you will assess diagnose, treat and provide advice, care and management for urgent and primary care patients, with either long term conditions, with diagnosed or undiagnosed undifferentiated illness. You will Support the delivery of policy and procedures, and provide leadership as required.

The clinician will act as a positive role model supporting junior staff with training and development including supporting joint consultations with trainees, whist working flexibly to provide safe clinical expert care. Theis clinician working in this role will demonstrate critical thinking and will be key to supporting clinical work across the wider healthcare team to meet the needs of patients.
