Northern Health GP Federation is recruiting to this post to work across the North Manchester Locality. (base TBC but likely to be Collyhurst)

Join us and be part of an innovative and exiting new service in Manchester where you will have the opportunity to develop and bring your ideas to continually develop and improve the team(s) approach and shape the new service offer for people in the City. You will be joining an already existing team which has been in place for the last 10 months, at the end of its pilot phase which is getting ready to roll out the service to another locality in North Manchester.


The existing team meet with and review people to identify their clinical and non-clinical support needs and develop informed care planning with the person and their carer(s). Each person have an assigned key worker within the team, regular multi-disciplinary care reviews and implementation of the care plans until they are better able to manage their illness and self-care with mainstream support. Local delivery of clinical and social care services, focussing on what matters to the person and their carer will be supported by rapid access to specialist advice, diagnostics and opinion through the networks that clinicians in the team will develop working in partnership with neighbourhood team colleagues and acute care colleagues.


The key to the success of this service is the need for each person to be committed to:

  • A focus on holistic care and support and the commitment to work with people and their carers to develop and implement care and support plans with them with a view to them better managing their illnesses and developing local support mechanisms longer term.
  • Personal and professional development as well as commitment to the continued development of the team and the HIPC service offer.
  • Integrated and multi-disciplinary team working, including a flexible approach to working across individual boundaries where required.
  • Challenging the status quo to improve service delivery and patient care and to support the Team Manager in their running of the service.


A flexible approach to employment for all posts will be considered as these are offered on a 12 month fixed term contract (with a view to extending beyond this into year 3 and beyond). This will include session, part time working and / or secondments from current positions where you are already working in the NHS.
