Band 7 Community Matron

15 hours – Fixed term for 12 months

Due to a successful internal secondment the Dorking Community Medical Team are looking for a full time Community Matron to join our caring and highly motivated multidisciplinary team.

The team deliver community based care to the population of Dorking and surrounding areas, covering a population of approximately 45,000 patients registered with the 4 GP practices in the area. Dorking provides beautiful scenery to your day and a great place to pause to have your lunch in the stunning hills.

The post holder will work within an integrated team to proactively manage, co-ordinate and support patients with complex social situations and/or multiple complex long term conditions, with responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life, ensure integration of all elements of care, increase patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admissions. The post holder will work across traditional organisational boundaries challenging inequities to co-ordinate and ensure health and social care needs of the individuals are met. The post holder will work collaboratively with other appropriate services to ensure patients are appropriately stepped up and stepped down from the community matron caseload.
