Band 7 Clinical Case Manager

Are you a highly professional and experienced RNC or RN who has excellent communication and interpersonal skills?

The role of the Clinical Case Manager is to lead on the clinical care of the child, ensuring the clinical care planning, equipment provision and care package is delivered effectively and efficiently.

Based in Guildford supporting a county wide service across Surrey

30 to 37.5 hour contracts to be considered

Full driving licence and access to a car – able to travel throughout Surrey and to the main tertiary hospitals in London and Southampton

As part of the role you must have community experience and knowledge of working with children with additional health needs, tracheostomies and long term ventilation. You will also be required to have experience of staff management and multi professional multi agency working. We ask that you are an effective team member, reliable, self-motivated with a huge passion for working with the children on the caseload.

FinalBanding for this position is dependent on experience

Key Duties:

  • Leading on clinical care for children on caseload

  • Maintaining close links with both community and tertiary based services and updating clinically where appropriate

  • Liaising with the Tertiary Hospital and local Peadiatricians about changes to the child’s clinical needs, and amending clinical care bundles as appropriate, updating the staff working with the child

  • Planning, providing and delivering competency based training to the carers and RNs

  • Providing training to school and nursery staff

  • Managing the out of hours On-Call on a rolling monthly basis

  • Providing clinical support to the carers and RNs and managing a caseload of staff

  • Providing 1:1 supervisions and appraisals to your staff and supporting team meetings

We will provide you with excellent training including:

  • Mentorship courses

  • Training provided by specialist hospitals, such the Royal Brompton Hospital in London

  • Training delivered in our designed training room recreates a child’s bedroom

  • Equipment training including ventilators, suction machine, a cot and mannequins
