Job summary

Braintree Primary Care Network Ltd is seeking a motivated and enthusiastic Coordinator to join our multidisciplinary team. The successful candidate will play a key role providing support across our three member practices and two hosted practices, serving a combined population of approximately 58,000 patients.

Working closely with GPs, nurses, and the wider primary care team, you will help deliver high-quality, patient-centered care to housebound and vulnerable individuals. You will be accountable to Braintree PCN Ltd and will collaborate across practices to ensure service flexibility, resilience, and effective cross-cover.

This role plays a vital part in supporting our GP practices by enhancing patient outcomes through the delivery of timely and effective healthcare.

Main duties of the job

The main duties of the role include:

Diarising and administrating/minuting PCN Clinical Learning meetings

Managing Protected Learning Time events and organising PCN-wide training as, delegated by Assistant BM.

Ensuring action log items are progressed

Supporting the induction of new members of staff

Ensure all PCN clinical and non-clinical staff have correct rotas entered on SystmOne and that these rotas are maintained correctly to ensure auditing is viable

Compiling newsletters and PCN-wide communications

Compiling reports and audits

Compiling impact and investment fund searches

Ensuring distribution lists are kept up to date

To communicate risks and issues

Monitoring relevant DES workstreams and completing a monthly tracker when necessary

Responding to patient enquiries

Sending referrals for PCN/ EA workload

Care Co-ordinator main duties include:

Work with all primary care professionals within the PCN to support patients with appointments, advice and general queries.

Work closely and in partnership with the clinical and non clinical teams and a host of other professionals

Support the clinical director and member practices in the delivery of the DES specifications, issued by NHS England

Provide coordination and navigation with the aid of digital tools for people and their carers across health and care services

JOB RESPONSIBILITIES

PCN practice initiatives

PCN and Practice Websites ensure information PCN and practice wide is kept up to date.

To develop and maintain an in-depth understanding of the PCN digital infrastructure.

To develop and implement digital transformation bids

To ensure all staff receive training on the digital tools within the PCN

To ensure the induction of all new members of staff receive training and resources on tools available

To remain up to date with the national and local PCN guidance.

Oversee and monitor telephony data across PCN and review data with member practices and collaboratively build a plan of action covering following 8 metrics:

Call Volumes

Wait before call abandoned

Calls abandoned

Call back requested

Time taken to answer calls

Missed call volumes

Call backs made

Average call length time

Arrange meetings with the ARRS teams/leads for, Clinical Pharmacists, Paramedics, Social Prescribers, and Care Coordinator. Discuss, agree and create Standard Operating Procedures for ARRS staff (SOPS).

SOPS to be signed off at Board and a review date to be agreed (would suggest every 3 years unless significant changes occur for example within the DES).

PCN Hub Administration

Enhanced Service Returns

Ensure SystmOne coding is accurately entered to enable practices to retrieve Enhanced Service claims.

General Hub Duties

Daily point of call for staff working within the Hub. Signposting as appropriate.

One off or long term admin tasks/duties delegated by Business Manager or her Assistant. Patient Initiatives Liaise and work with First Stop project organising local ICT lessons and digital equipment which can be loaned to patients. Tackling digital exclusions and patient access for patients with learning difficulties and health conditions ensuring PCN compliance and assisting PMs within member practices.

INT Leadership Group

Be key communicator with Mid Alliance INT regarding integration with secondary care required under DES alongside PCN INT Lead.
