Job summary

Venn Primary Care Network (PCN) is seeking a Fully Qualified Advanced Clinical Practitioner (ACP) and Independent Prescriber to join our established Home Visiting Team.

About the role:

  • You will visit housebound patients across our network of 9 GP practices, delivering high-quality clinical care.
  • A full UK driving licence and access to your own vehicle are essential, as the position involves travel across Hull and East Riding.

Main duties of the job

After completing the necessary clinical supervision, the post-holder will work as an independent practitioner, delivering high-quality care to patients with undifferentiated and undiagnosed conditions. This includes:

  • Face-to-face assessment, examination, diagnosis, treatment, management, referral, review, or discharge, in line with the Competency Framework.
  • Utilising a broad range of clinical skills and advanced knowledge to ensure safe and effective patient care.

Key Responsibilities

  • Home Visiting Service: Provide planned visits to housebound patients in the community as part of the primary care team.
  • Timely Treatment: Initiate treatment plans promptly to ensure patients receive appropriate care without delay.
  • Patient Reviews: Conduct reviews for patients who are unable to attend the practice, ensuring continuity of care.
