Job summary
Tadcaster & Selby Rural PCN is looking for an experienced and motivated Advanced Clinical Practitioner (ACP) to take on a pivotal leadership role in our care home service.
This is a fantastic opportunity for a confident, autonomous clinician to work across three care homes within our PCN, supporting approximately 130 residents with complex needs. You'll lead weekly ward rounds, manage acute and chronic conditions, support admission avoidance, and play a central role in ensuring high-quality, proactive care.
The successful candidate will be the key clinical link between care homes and GP practices and will help shape a service that makes a meaningful difference to some of our most vulnerable patients.
This role is for 4 sessions per week (2 days) based within the local care homes/Practices.
This role is based on 4 sessions per week (2 days) to be spent in the Care Homes completing the weekly ward round, updating care and action plans as well as supporting acute medical issues on the day. You will be working and prescribing autonomously. Clinical supervision will be provided a named clinician on behalf of the PCN and individual Care Home supervisor (based on the care home assignment) will be available regularly.
Main duties of the job
The role aims to reduce ad-hoc demand on services (outside of the weekly ward round) through advanced planning and coordination of care across multiple disciplinary teams. The role also provides greater capacity for General Practice whilst ensuring Care Home patients receives an excellent level of care.
The role will act as the main clinical lead and resource between General Practice and the Care Home.
The role will also support any future care home / assisted living facility that opened within the PCN boundaries in the future.
The role will work closely with the PCN Practices, PCN Manager and other AHP's to achieve the objectives of the PCN. You may be asked to support the mentorship of junior colleagues or other roles within the Practices or PCN (in Care Home specific roles).
You will also act as a clinical leader in the delivery of the Care Home services within the PCN or Practice, whilst ensuring the patients' needs are a priority.
You may be asked to help support other workstreams to fit the needs of the business.
