Job summary
In
Benfleet PCN the role will be focused on two days of clinial sessions within
two of the seven practices in the PCN with the other two days focused on our
care home patients (Benfleet has eight care/reidental homes in total). One of
these days to complete a weekly ward round for one of our largest nursing homes
and the other to focus on advanced care planning for our care home patients,
incuding completing PEACE documents, DNACPRs, convert medication documentation
and anticapatory medication documentation.
It is preferred
if the individual in this post is an imdependent prescriber.
To note, this role is
for maternity leave cover on a fixed term contract for 9-10 months. Beginning of
May 2026 to March-April 2027.
Main duties of the job
Two clinical days in practice, one day completing a weekly ward round for one of our care homes and one day to complete ACP for our care home population.
About us
Benfleet PCN has around 50,000 patients from the borders of Rayleigh to Leigh-On-Sea. Our PCN includes 8 care homes, around 1000 housebound patients and 7 GP practices.
The PCN team is made up of a broad team of 42 individuals working in practice, remotely, out in the community or on a hybrid basis. We are widely known in MSE ICB as an examplarar PCN having a very supportive workforce and drive to support training and ongoing learning.
