Job summary

We are seeking an experienced Advanced Practitioner to join our busy team. You will work autonomously to assess, diagnose, and manage patients across a range of conditions, including acute, chronic, and urgent presentations. This role combines clinical expertise with leadership, education, and service development responsibilities.

Main duties of the job

Conduct comprehensive patient assessments and develop management plans.

Prescribe medications independently (NMP qualification required).

Provide leadership and mentorship to clinical staff.

Contribute to quality improvement, audits, and service innovation.

Support education and training within the PCN.

Collaborate with multidisciplinary teams and external agencies.
