Job summary
The Care Coordinator supports effective service
delivery within the Primary Care Network by organising and streamlining care
coordination processes, managing administrative systems, and ensuring smooth
communication between teams. This role involves proactively supporting patients
and staff with complex needs through logistical planning, working closely with
practice teams, external providers, and the wider PCN to ensure services are
delivered efficiently and experiences are improved.
Main duties of the job
The Care Coordinator is responsible for managing administrative and coordination tasks across the PCN, including rota management, meeting organisation, data handling, and inter-agency communication. The role supports smooth information flow between services, maintains accurate records using systems like EMIS, assists with audits and reporting, organises events and training logistics, and ensures compliance with data protection standards. The postholder also plays a key role in improving internal workflows, preparing documentation, and supporting the wider PCN team with service development and operational delivery.

We are a Primary Care Network (PCN) of five practices working together
with a range of local providers, including primary care, community services,
social care, and the voluntary sector, to offer more personalized, coordinated
health and social care to their local populations.
Our PCN comprises of 35k+ patients over the five practices, making us
large enough to provide resilience and support the development of integrated
teams.

- Methodical, approachable, flexible, and team-oriented; committed to service quality
Desirable
- Further training in project support, data entry, or coordination
- Experience supporting project management, audits, or PCN delivery
- Experience using EMIS or similar systems; report writing skills
- Familiarity with PCN structures, referral pathways, or QOF processes
