Rural Warwickshire North PCN is delighted to offer an exciting opportunity for a newly qualified General Practitioner to join our team. We are seeking a GP who has obtained their Certificate of Completion of Training (CCT) in General Practice within the past two years and has not yet held a permanent role in General Practice.

Main duties of the job
Provide high-quality, patient-centered care across multiple practices within the PCN.Conduct consultations, assessments, and follow-ups for patients with a variety of healthcare needs.Collaborate with multidisciplinary teams to develop and implement patient care plans.Contribute to the delivery of services aimed at improving patient outcomes and overall health.Support the implementation of quality improvement initiatives and clinical governance standards.Participate in training and development opportunities, enhancing personal and professional growth.Ensure accurate patient records are maintained, following best practice guidelines.

- Diagnose, treat, and manage both acute and chronic conditions, with a focus on continuity of care.

Collaboration with Multidisciplinary Team:
- Work closely with other GPs, Nurses, Pharmacists, Social Prescribers, and Allied Health Professionals to deliver comprehensive care.

- Take an active role in improving clinical pathways and delivering quality care initiatives.

Quality Improvement and Compliance:

- Experience working in Primary Care or within a GP Practice setting.
