Job summary
Shore Medical is seeking a Palliative Specialist Community Nurse to provide specialist palliative and end-of-life care to patients within the Shore Medical PCN, ensuring holistic, patient-centred care that supports individuals and their families during the end-of-life journey.
The role involves working collaboratively with GPs, district nurses, community teams, and other healthcare professionals to deliver high-quality care, promoting dignity and choice at the end of life.
Main duties of the job
- Lead on the assessment, planning, implementation, and evaluation of end-of-life care for patients within the PCN.
- Manage complex cases, providing specialist advice to primary care teams.
- Coordinate care with GPs, district nurses, hospices, and other community services.
- Support anticipatory care planning and facilitate discussions around advanced care planning.
- Prescribe and manage medications for symptom control (if non-medical prescribing qualification is held).
