HealthCare Assistant - Frailty

Job summary

Shore Medical is recruiting an experienced Health Care Assistant to be part of the Frailty Team providing high quality and forward-thinking nursing care to meet the holistic healthcare needs of the Practice population.

The role will involve implementing treatment programmes with individual patients and/or carers in their own homes as delegated and directed. Providing high standards of care encompassing a rehabilitative approach where necessary, and report back as agreed.

Main duties of the job

Primary duties and areas of responsibility

1.Is accountable to the Nursing Lead and the Primary Care Network for their actions in undertaking delegated work.

2.Provides support to other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole.

3.Refers to more senior members of the Primary Health Care team as appropriate.

4.Works within all relevant practice policies and procedural guidelines for example: infection control, chaperoning.

5.Is required to keep competencies up to date and partake in ongoing training and appraisal process for continuing professional development.

6.Give specific treatments under Patient Specific Directions where appropriate and if deemed competent.

Clinical Responsibilities

1.Venepuncture, BP checks, height, weight, BMI, urinalysis, ECG

2.NHS health checks

3.Long term condition/Chronic disease health checks

4.Flu & Covid vaccination (following approved training and competence checks

5.Diabetic Foot Checks (following approved training and competence checks)

6.Dressings (following approved training and competence checks)

7.Smoking Cessation (following approved training and competence checks)

8.Responsible for the correct and safe management specimens including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements

9.Safe management of sharps procedures including training, use, storage and disposal
