PCN Clinical Pharmacist
We are looking to recruit to the post of Clinical Pharmacist, to
work within our Primary Care Network multidisciplinary healthcare team.
You will be primarily based within a number of
our PCN practices.
The ideal candidate should be flexible with a proactive approach to work
and possess excellent communication skills.The
role requires face2face attendance for a significant proportion of the
activities.
Main duties of the job
Work as part of a multi-disciplinary team in a patient facing role to
clinically assess and treat patients using expert knowledge of medicines.
Have completed the
CPPE leaning pathway for PCN pharmacists.
Be a prescriber, or
happy to train to become a prescriber.
Work with and
alongside the PCN team to; take responsibility for the management of our care
home patients. This will include undertaking home ward rounds, conducting
assessment of illness, completing structured medication reviews, proactively
managing people with acute and chronic illness, complex poly-pharmacy and on
high risk medications. Produce care plans in line with the GP's and patients
families.
The post holder will work within a PCN Pharmacy Team to support practices with a variety of activities including, carrying out SMRs, supporting management of shared care agreements, reviewing discharge letters and ensuring the medications are updated. Work across the PCN to help meet the prescribing elements of the PCN DES.
The post
holder will work with a diverse range of people from different cultural and
social backgrounds. The ability to work confidently and effectively in a
varied, and sometimes challenging environment is essential.
The
successful candidate will have excellent interpersonal and communication
skills, and be organised, patient and empathetic. They will have experience of
working in health, social care or other support roles including direct contact
with people, families or carers
About us
We are a friendly and supportive
well-established Wolverhampton based General Practice Federation.We
consist of two primary care networks. This role will be based with our Unity
West PCN consisting of 6 practices and 46500 patients.
We have an established PCN
management team of 2 GPs, and ANP and a PCN manager. The West team already
consists of a number of clinical pharmacists, all of which are prescribers. We
also employ a number of other ARRS staff.Our senior management team
are well versed in CP / GP / Nurse and HCA training and candidate development.

Essential
- Masters degree in pharmacy (MPharm) (or equivalent)
- Specialist knowledge acquired through postgraduate diploma level
- or equivalent training/experience
- Independent prescriber or working towards/intent of gaining independent prescribing qualification.
- Mandatory registration with General Pharmaceutical Council.
- Minimum of 2 years post qualification experience.
- In depth therapeutic and clinical knowledge and understanding of
- the principles of evidence- based healthcare.
- Minimum of 2 years post qualification experience.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
- Previous experience of working in a PCN or General practice
- Already completed CPPE
- An appreciation of the nature of GPs and general practices
